NYS MWBE Certification: Required Documents for Sole Proprietorship
Review the list of documents needed to submit a New York State MWBE certification application as a sole proprietorship:
- Certification Affidavit (Part of application; Signed and Notarized)
- Personal Net Worth Affidavit, Attachment A (Part of application; Signed and Notarized)
- Personal Net Worth Worksheet, Attachment B, if applicable
- Proof of US citizenship or permanent resident alien status
- Proof of minority group member status, if applicable
- Proof of gender, if applicable
- Capitalization Proof of Business Capitalization (in the form of money, property, equipment, or expertise) if applicable
- Federal and State Individual Income Tax Return
- Personal Tax Returns Federal and State Income Tax Return
- with (Schedule C) for the applicant business
- Form 1099s issued by the applicant business
- All W-2s issued by the firm as well as filed W-3
- Bank signature cards or a letter from the bank
- Resumes of all owners, directors, officers, and other key employees of the applicant business
- All Licenses, Permits, Certifications Copies of all licenses, permits, certifications, and/or accreditations.
- Business Certificate
- Proof the applicant business is actively conducting business in New York State
- Current leases and/or deeds
- Third-party agreements
- Narrative of how business was started
- All Contracts, Invoices, Purchase Orders with Proof of Payment identifying NAICS codes requested
- Optional: Written request for exemption from disclosure regarding trade secrets
- Optional: Addendum for MWBE Certification with County of Erie and City of Buffalo; Joint Certification Committee
- Optional: Addendum for MWBE Certification with New York City Department of Small Business Services
- Optional: Addendum for MWBE Certification with Port Authority of New York and New Jersey Additional supporting documents not listed
